Remember, folks, a lot of people will look at your report and copy it to fill out their own (they shouldn't be, though, instead they should use a standard form), so if your formats are wrong, bad habits are going to spread quickly! Consistency in following the standard time and report formats is important, so please make sure you read through the thread Zerowinged linked to and update your reports to follow it.
